Please inform your scheduling team if you’ve had a positive COVID-19 test within 90 days of your appointment and bring a print out of your test result or printed photo of your home test, if available.

Your appointment schedule may need to be modified if you recently tested positive for COVID-19, develop symptoms or have a close contact exposure to someone with COVID-19. Contact us to discuss next steps if any of these concerns arise.

You may be asked symptom screening questions when speaking with our scheduling specialists, through text messages or in messages sent to your online patient account. We ask that you opt-in to receive these automated notifications and communications to help us stay in touch with you prior to your appointments.
